About Infertility

Infertility is defined as the inability to conceive despite having frequent, unprotected intercourse for at least a year. Infertility affects 10% to 15% of couples in the United States.

Infertility can be caused by a combination of circumstances that impede pregnancy. Fortunately, several safe and efficient treatments can dramatically increase your chances of conceiving.

Common Medication
Treatment Options

Infertility treatments, according to the Mayo Clinic, are dependent on the following:

  • The cause of infertility
  • The length of time you have been experiencing infertility
  • Your age and the age of your partner
  • Your personal preference

It is important to note that some causes of infertility cannot be treated. Couples who are unable to conceive naturally can often obtain a pregnancy via the use of assisted reproductive technology (ART), such as in vitro fertilization.

  • Medical history of reproductive conditions like endometriosis, fibroids, or gynecological cancer
  • Trying to conceive with a medical history of sexually transmitted infections such as HIV/AIDS, and human papillomavirus (HPV); or complications from STDs like pelvic inflammatory disease (PID)

REs may conduct additional tests to assess reproductive health, including:

  • Cervical mucus exams
  • Hormone testing, including luteinizing hormone (LH tests)
  • Blood tests to screen for human papillomavirus (HPV), hepatitis, and HIV/ AIDS

Obstetricians and gynecologists are very similar - but there are some differences between the two.

Obstetrics:

Obstetrics deals specifically with all aspects of pregnancy, including prenatal and postnatal care. Obstetricians are responsible for delivering babies and treating pregnancy-related issues, such as fetal distress and ectopic pregnancies (the fetus grows outside of the uterus). Obstetricians, along with endocrinologists, can also provide therapy to help with fertility and pregnancy issues.

Gynecology:

Gynecologists specialize in the female reproductive system and treat a broad array of women's health conditions.

Gynecologists help women with reproductive issues from their first period through menopause. Gynecologists will also perform a number of regular tests and exams, such as:

  • Pap smear (to screen for cervical cancer)
  • Breast exams (screening for breast cancer)
  • Pelvic exams (screening for ovarian cysts, uterine fibroids, STIs, and cancer)

Gynecologists can also be consulted for:

  • Contraceptive/ birth control counseling
  • Endometriosis treatment
  • Abnormal/ irregular menstruation
  • Polyps in the reproductive system (usually in the cervix or vagina)
  • Infection (such as UTIs or STIs)

Most doctors will undergo training to specialize in both obstetrics and gynecology. This allows the provider to offer more complete and comprehensive care to patients. Providers who specialize in both obstetrics and gynecology are known as OB/GYNs.

Gynecologists are doctors who specialize in women’s health and the female reproductive system, which includes the vagina, uterus, ovaries, and fallopian tubes. Most gynecologists are also doctors of obstetrics (practices associated with childbirth, pregnancy, and postpartum care), which is why gynecologists are often called OB/GYNs. Gynecologists can treat issues related to menstruation, fertility, STIs, menopause, and hormones.

The American College of Obstetricians and Gynecologists (ACOG) recommends that girls first see an OB/GYN between the ages of 13-15. Many issues and concerns before the age of 13 can be handled by a primary care physician. The first visit with an OB/GYN may just be a talk about development. There may be personal questions about puberty, menstrual periods, and sexual activity. Most girls will not need a Pap smear or pelvic exam on their first visit unless they are experiencing abnormal pain or bleeding.

Clomiphene (generic Clomid) is a medication used to stimulate ovulation in women who are struggling with infertility. It works by stimulating the production of select hormones that support the growth and release of a mature egg (a process called ovulation).

Clomiphene is usually prescribed by a primary care physician, OB/GYN, or fertility specialist. It is available in its generic form or under the brand name Clomid. It can be prescribed on Sesame after an online fertility consult, at your provider's discretion. For more information, talk to a doctor about whether clomiphene is right for you.

Clomiphene helps you to produce an egg by tricking your body into thinking that your estrogen levels are lower than they are. This causes the pituitary gland to secrete more follicle-stimulating hormone (FSH) and luteinizing hormone (LH). These increased levels of FSH stimulate the ovaries to produce an egg follicle (or multiple follicles). Higher levels of LH stimulate ovulation so that the egg follicle(s) can be released so they can be fertilized.

Clomiphene is generally taken five days in a row toward the beginning of your menstrual period. Ovulation doesn’t occur while you’re taking the medication, however, from your first dose your body begins working towards producing an egg. Ovulation generally occurs 5 to 10 days after taking the last clomiphene pill.

Because clomiphene has been shown to increase risk of ovarian cancer, it should not be taken for more than 6 menstrual cycles in a row. Talk to your provider about the proper dosage of clomiphene and the risks associated with taking it.

Many people who use clomiphene experience no adverse effects at all. However, there are some side effects that you should be aware of. The most common of these are:

  • Flushing (a feeling of warmth)
  • Upset stomach or vomiting
  • Breast tenderness
  • Headache
  • Abnormal vaginal bleeding

Talk to your doctor if any of these are severe or do not go away.

While a severe adverse reaction to this medication is uncommon, it can occur. Get immediate medical attention if you experience any of the following:

  • Vision problem (such as blurred vision, double vision, or visual spots or flashes)
  • Stomach pain or swelling
  • Unexplained weight gain
  • Shortness of breath

Please note that this is not a complete list of all possible adverse reactions or precautions. Seek medical attention right away if you experience an upset stomach, vomiting, blurred vision, vision impairment, and stomach pain or swelling.

Beause clomiphene can cause blurred vision, do not operate a motorized vehicle or heavy machinery until you know how the medication affects you.

As with all prescription medication, be sure to inform the prescribing doctor about any medical conditions you have been previously diagnosed with and any medication/ supplements you are currently taking before taking clomiphene. Clompiphene can interact with other forms of medication, causing potentially serious side effects. In addition, let your doctor know if you are pregnant or plan on becoming pregnant before starting treatment with this medication. For more information please visit the National Institutes of Health’s DailyMed webpage for clomiphene.
