Primary Care Visit & Intro to Direct Primary Care
50-minute appointment
New patient primary care visit and introduction to direct primary care, 50 minutes.
In order to take advantage of your full 50 minutes, you must arrive early enough to complete your intake paperwork (15-20 minutes). Those arriving late will not be accommodated and will be advised to rebook. Refunds are not given in these cases.
NOTE: we DO NOT provide services related to disability assessment and paperwork, worker's compensation claims, and/or controlled medicine prescriptions for new patients. Additionally, all KCDPC providers and staff reserve the right to refer patients on to higher-level care as appropriate and will certainly do so, potentially before the end of the visit, for those needing emergency or urgent care services; the visit fee will not be refunded in this situation.
Those interested in enrolling in our direct primary care (DPC) program will receive a credit towards their enrollment fee and first month's membership equal to the amount paid to Sesame.
For those not enrolling in our DPC program, refills stemming from this visit will be provided on a case-by-case basis and will not be given more than 90 days into the future without an in-person visit.
About the doctor
Dr. Hazen Short went into medicine to help patients by building meaningful relationships. Family Medicine encompasses his passion in health care: wellness, prevention, and continuity of care. His professional interests include evidence based medicine, mental health care, hospital medicine, and pediatrics. Originally from Manhattan, KS, Dr. Short attended Kansas State University for his undergraduate degree in Biology and minor in Leadership Studies. He then attended the University of Kansas School of Medicine in Kansas City. He completed residency at Research Medical Center located in Kansas City, MO. In addition to working at KCDPC, he works part time as an emergency physician in critical access hospitals. He volunteers as a Clinical Assistant Professor for the University of Kansas, and is an eagle scout, a singer, plays cello and guitar, and loves to play ultimate frisbee, soccer, and disc golf. He is a father and a husband who loves to spend time with family.
