Both hydrochlorothiazide and chlorthalidone are thiazide-like diuretics used to treat high blood pressure. Chlorthalidone stays active in the body longer and is more potent per milligram. Hydrochlorothiazide is active for a shorter amount of time and is widely used in fixed-dose combination blood pressure pills.

High blood pressure can be treated with several classes of medications, including:

For high blood pressure in adults, the typical starting dose is 12.5 mg to 25 mg taken once per day. For swelling, starting doses range from 25 mg to 50 mg per day, sometimes given in divided doses. The total daily dose for edema should not exceed 200 mg.
Hydrochlorothiazide can interact with lithium, other blood pressure medicines, alcohol, narcotics, NSAIDs like ibuprofen or naproxen, antidiabetic drugs like insulin, bile acid sequestrants, and corticosteroids.
Do not take hydrochlorothiazide if you cannot urinate (anuria) or have a known allergy to the drug. Tell your provider if you have a history of kidney or liver disease, gout, lupus, severe electrolyte imbalances, diabetes, or high cholesterol.
