Emerald Sonico, FNP-C, CDP is a board-certified Family Nurse Practitioner who specializes in family medicine, chronic disease management, geriatric care, hospice and palliative care, home health, and women’s wellness, and who has been practicing for over six years as a Nurse Practitioner and more than 15 years in nursing overall. She is a Certified Dementia Practitioner and a member of various multidisciplinary care teams throughout her career. She offers care in the following languages: English, Tagalog. She is able to treat symptoms of hypertension, diabetes, chronic pain, weight loss, urinary incontinence, depression, fatigue, sleep disturbances, UTI, mood changes, and chronic muscle aches. Her passion for healthcare began with her early work in pediatric oncology, and has deepened through her experience caring for seniors and homebound patients. She finds it incredibly fulfilling to connect with patients on a personal level, helping them manage complex conditions while maintaining dignity, independence, and quality of life. Emerald brings a calm, compassionate approach to care and is known for her attention to detail, cultural sensitivity, and commitment to building long-term patient relationships. With experience in both traditional and home-based settings, she ensures every patient receives thoughtful, holistic, and individualized care.
