About Bipolar disorder

Bipolar disorder (BD) is a mental illness that causes extreme, unpredictable mood swings. Individuals experience periods of emotional highs (manic episodes or hypomanic episodes) and lows (depressive episodes). These mood episodes can last for days or even weeks, affecting relationships with loved ones, decision-making and daily life.

Bipolar disorder symptoms can vary and may include:

  • Mania or hypomania (racing thoughts, impulsivity, increased confidence, decreased sleep, increased energy, trouble concentrating and heightened euphoria or irritability)
  • Depression (persistent sadness, fatigue, loss of interest in everyday activities, trouble concentrating, or suicidal thoughts)
  • Poor decision making
  • Changes in appetite and weight
  • Extreme shifts in mood and energy levels

Bipolar disorder is a lifelong mental health condition. However, it can be managed effectively with proper medical care and support. If you or a loved one is exhibiting symptoms of bipolar disorder like the ones listed above, seek medical advice from a healthcare professional.

Treatment Options

Treatment for bipolar disorder often involves a combination of medication and psychotherapy. Treatment also depends on a person’s symptoms, age, and how much it affects their daily life.

Below are potential treatment options that may be recommended or prescribed by your Sesame provider. Please note that doctors and clinicians on Sesame will not prescribe controlled substances.

  • Antipsychotics: These medications help treat symptoms of bipolar disorder like mania and depression. Examples include quetiapine, olanzapine, risperidone, aripiprazole, and lurasidone.
  • Antidepressants: Antidepressants like fluoxetine and bupropion may be prescribed to treat depressive episodes, but can also trigger mania and should be used cautiously.
  • Mood stabilizers: These medications are often prescribed to treat extreme mood swings. Common mood stabilizers include lithium, valproate, carbamazepine and lamotrigine. Please note that some mood stabilizers may require monitoring via lab work, which your provider may order if needed.
  • Cognitive-behavioral therapy (CBT): CBT is a type of psychotherapy that can help you identify unpleasant thoughts and behaviors and replace them with positive coping strategies. This form of therapy has been shown to decrease mania and depressive symptoms in bipolar patients.

There are several main types of bipolar disorder: Bipolar I, Bipolar II, and Cyclothymic disorder. Common symptoms associated with each type of disorder are listed below:

  • Bipolar I disorder is considered to be the most severe type of bipolar disorder and involves at least one manic episode (extreme energy, reckless behavior, etc.) that lasts for at least 7 days or is severe enough to require hospitalization.
  • Bipolar II disorder is usually characterized by depressive episodes. It usually involves at least one major depressive episode and at least one hypomanic episode (a milder form of mania that lasts at least 4 days).
  • Cyclothymic disorder involves numerous periods of hypomanic and depressive symptoms that persist for at least 2 years. While symptoms aren’t as severe as Bipolar I or Bipolar II symptoms, these mood swings can affect your quality of life.

Please note that there are also other bipolar and related disorders with symptoms that don’t fully fit into the above categories but still cause significant distress.

Mania is a period of extreme mood elevation that often includes euphoria, racing thoughts, impulsivity, and even hallucinations. It can sometimes result in hospitalization.

Hypomania is often said to be a milder form of mania. It doesn’t cause psychosis like mania does, but it can still disrupt your life.
