Costco Fertility Medication Discounts
Access discounted fertility medications through Costco Specialty Pharmacy.
For patients
Patients should complete these steps before their provider sends a prescription:
1. Create your Costco Pharmacy account
Click here to create your account or sign in. Need help? Follow this step-by-step walkthrough.
2. Complete the patient intake form
Fill out Costco’s patient intake form so that your fertility medications can be processed. You’ll need to enter your provider’s name and address for this step (NPI is optional).
3. Have your provider submit your Rx
After completing steps 1 & 2, share this page with your fertility provider so they can register and send prescriptions to Costco Specialty Pharmacy.
For providers
Providers should complete these two steps after their patient has set up their Costco Pharmacy account and completed patient intake:
1. Complete provider registration
Before submitting any prescription, you'll need to complete this one-time clinician registration form.
2. Confirm that your patient has set up their account
Before prescribing, confirm that your patient has created their Costco Pharmacy account and completed their patient intake form.
3. Submit the prescription
Once registration is complete and the patient is ready, submit the prescription using one of the following methods:
